Located in Green Springs, Ohio, in the heart of Northwest Ohio, St. Francis Health Care Centre provides compassionate care for the elderly and physically challenged.  We are a healthcare ministry under the sponsorship of the Franciscan Sisters of Our Lady of Perpetual Help based in St. Louis, Missouri.  As a Catholic facility, we respectfully offer those we serve care, hope, and dignity in a joyful and compassionate manner.  Utilizing a wholistic approach to health care, we are committed to excellence in teaching those we serve to adapt to their disabilities and to maintain the highest level of independence.  We strive to meet the changing needs of the physically challenged and of the elderly by continually assessing the services we provide.  Seven integrated medical and physical rehabilitation programs are offered within its four units:

• A 36 bed Long Term Acute Care Hospital (LTACH) for acute physical rehabilitation and medically complex patients.
• A 150 bed Nursing Facility (NF) for general physical rehabilitation, subacute medical patients, long-term residents and short stay convalescence.
• A Therapy Center with physical, occupational, speech/hearing, and aquatic therapies for inpatients and outpatients
